Nurses Notes:
 
We are here to help facilitate the health care needs of your student, in conjunction with their doctor's. In order to achieve optimal care and health in the school setting, please have all paperwork completed and returned by the beginning of the school year. This paperwork is required by law, and we are unable to train and delegate health care tasks to the teachers if paperwork is not completed, by the physician. A chart is available to help you navigate the paperwork that is required for your student. It is located to the right, with the forms you will need.

To help keep our students healthy, they are not permitted to be at school until they are 24 hours fever & vomiting free.


MEDICATION GUIDELINES
 
vThe parent/guardian must sign a Permission to Administer Form before any medication can be administered at school.

v
Any Prescription medication will require the physician signature on the Permission to Administer form before any medication can be administered.
vParent/Guardian should not fill in physician’s information.

v
Medications cannot be transported on the bus or in student’s bookbag.

v
All medications must be signed into the school office by the student’s parent/guardian.

v
Medications must have a current pharmacy labeled container which includes the student’s name, physician name, name of medication, strength, dosage, time interval, and route.

vIf the prescription medication is changed during the school year, a new authorization form is required. Both physician and parent/guardian must sign the new form.

vNo verbal or telephone orders can be accepted.

vBy law, back up medication (epinephrine autoinjector) must be kept on school property at all times for emergency and field trips.

vParents/guardians have 5 days after the end of the school year to pick up all unused medication or it will be destroyed.
